In addition to providing hospice care, Community Hospice & Palliative Care offers several programs that help improve quality of life, such as Community PedsCare, a nationally recognized pediatric palliative and hospice program for children with life-threatening and life-limiting conditions; Community Hospice & Palliative Care Veterans Partnership, a program providing specialized care and support to address the unique needs of Veterans; Community Palliative Care, a consultative service for people with serious and complex illnesses that helps them make important health care decisions; and Community Care Choices, a CMS research model to help people with advanced illness find greater access to comfort care and support.
The Community Hospice & Palliative Care Neviaser Educational Institute provides education and other resources for health care professionals and the community about dealing with serious illnesses and their impact.
