Care Home By RNs
We are a group of Nurses and Caregivers who founded and manage this facility to provide transition of care to community-based post-hospital care for patients with complex care needs (e.g., stroke, TBI, MVA), who may be on a tracheostomy, on a ventilator breathing machine, may have deep stage 3/4 wounds, on PEG tube feedings, and others, in a home setting more conducive to rehabilitation.
Congregate Living Health Facilities (CLHFs, pronounced “cliffs”)) are residential medical care facilities that provide inpatient care in a home-like, communal environment. Levels of care include skilled and subacute levels.
